We welcome teens over the age of 16 to our practice. Teens changing from their pediatrician and their parents should be aware that we are an adult primary care practice-meaning we treat all of our patients as adults and involve them directly in their care. Teens (and their parents) should be comfortable with being treated as an adult for their care. This assumes that a teen will be comfortable with a history and physical exam being taken without a parent present, and that the exam may include a breast and gynecologic exam.
For patients under the age of 18 seeking ANY medical care or treatment with Lisa Larkin, MD, and Associates, a consent to treat form must be signed and on file prior to receiving any care or treatment. This consent is to be signed by a parent or legal guardian. In the event of a divorce, the parent with the legal custody or the residential parent of the minor should sign the form.
There are special circumstances when a minor may provide consent for treatment (without parental or guardian consent). These include:
- For an examination for the purpose of gathering physical evidence of alleged sexual offense
- For the diagnosis or treatment of any venereal disease by a licensed physician
- For the diagnosis or treatment of any condition that it is reasonable to believe is caused by drug abuse, beer or intoxicating liquor
- To be given an HIV test for the diagnosis of AIDS or an AIDS-related condition
